CDS Employer Enrollment – MDCP
Before Astrocare is able to process payroll for a CDS Employer, the below must be completed, signed and submitted. To avoid any delay in payroll processing, please ensure that all items are completed, signed and submitted prior to the authorization effective date. Any delay in submission may cause delay in payroll processing.
- Please provide YTD wages for employees that have worked CDS in the current year
- 3rd Party Documentation of both EIN and TWC accounts
- EVV Required Training Checklist
